Job Description:

Housing Development Services, Inc. (HDSW) seeks a Certified Peer Specialist Peer Care Manager to join our dedicated team in the Community & Social Services department. This role focuses on utilizing lived experiences to provide support, advocacy, and coordination of services to individuals facing housing instability due to mental health challenges or disabilities. The successful candidate will work collaboratively with clients to develop personalized recovery plans, promoting empowerment and self-determination in a supportive community setting. This role is crucial in fostering a peer-support environment and helping clients to navigate both the healthcare and housing systems effectively. Provide peer support and personalized recovery coaching to clients struggling with mental health and housing instability. Develop and implement tailored care plans that respect individual goals and preferences while promoting independence and community integration. Advocate for clients' needs and rights in various settings, including healthcare meetings and housing negotiations. Coordinate with healthcare providers, housing authorities, and other agencies to streamline access to services and resources. Facilitate group sessions and workshops to educate and empower clients about self-care and community resources. Document client interactions and progress accurately to ensure compliance with organizational and regulatory standards. Strong interpersonal and communication skills Ability to apply empathy and active listening in peer support settings Experience in crisis intervention and conflict resolution Proficiency in care coordination and client advocacy Knowledge of mental health systems and housing resources Certified as a Peer Specialist through a recognized certifying body Additional training or certification in mental health or social services is advantageous
